Casefic is lovely -- maybe something from John's time in the military comes back to haunt him, and Sherlock has to investigate? The graffiti artist from Blind Banker has a mystery to be solved, so he approaches Sherlock? Or Sherlock slips back into his drug abusing ways and his family and friends have to rally to help him? I love the sibling rivalry and concern of Sherlock and Mycroft (maybe another sensitive government project needs secured? or they're reminded of an incident in their childhood?), but no incest, please. If you go a kinky route, I admit I see Sherlock as the submissive one. If you have a hankering to go the crossover route, I've wondered about how Sherlock's characters would interact with those from Primeval, Keen Eddie, and Muppets (Muppets make anything better).
