I love dialogue, clever banter, in-characterness, plot twists, fix-its, pining, misunderstandings, outsider POV, UST that gets resolved, and all the punctuation in the right places. A well-done casefic will always win my heart, especially if the case serves to further the Johnlock agenda. True love through adversity is also a surefire crowd pleaser.
I like Victorian-era Holmes and Watson best when they pine and are in denial both to themselves and each other but end up together at the end anyway. I have a soft spot for BBC John and Sherlock when one of them pursues the other, even if only subtly. I love them in any era if they are damaged but make each other better. No dark!Sherlock or dark!John though, please.
Some more specific scenarios if you still need inspiration: Negative repercussions of John and Sherlock (or Holmes and Watson) being together; ACD Watson is left to raise his child alone after Mary dies; Holmes and Watson having a private ceremony of some sort to celebrate their relationship; John gets jealous of Irene returning; going to therapy (either John, Sherlock, or both); post-series 4 from Harry Watson's POV when John shows up one day to dump Rosie on her.
