I love first-time and absolutely adore the five times trope (idk what it is about this fandom that makes it so glorious!) Anything where Sherlock and John are having a banter and being a bit snarky but funny is generally a win in my books, especially if they're awkwardly trying to tell each other how they feel. Equally I love it when they don't NEED to say anything, they just understand on an instinctual basis. Hurt/Comfort is great, esp if one of them is ill or injured and the other looks after them. I'm pretty much a walking cliché when it comes to Sherlock and John :D
Just an fyi, I hate kid!fic and mpreg an impossible amount.
